Important FAQs on the treatment of pigment and age spots

In principle, CO2 laser treatment is not painful. However, if desired, the patient can be pre-treated with a local anesthetic ointment. For smaller areas, the treatment is completely painless as a special icing spray is used.

Following the treatment, a cream should be applied to accelerate wound healing. I will of course be happy to advise you on this as part of the treatment.

The CO2 laser treatment is suitable for all those who suffer from one of the problems listed below:

After the skin treatment, it is quite normal for redness and swelling to occur and for crusts to form on the treated area. Slight itching may also occur during the healing process. However, this is merely a sign of healthy healing and the discomfort should subside a few days after the treatment.

As a rule, a single application of the laser is enough to trigger a noticeable improvement in the skin. However, this cannot be said across the board, as every patient is different! After the procedure, it is essential that you take intensive care of your skin and do not expose it to sunlight. You will notice the first improvements after just a few days. More pronounced wrinkles usually take a little longer to heal. Here, the first improvements can usually be seen and felt around ten days after the laser treatment. As the skin is renewed from the inside out, you can often notice positive results even months later.

An analgesic cream is applied before the treatment, which is normally sufficient. For further relief of irritation or itching that may occur after treatment, I recommend special ointments and creams.

Prolonged exposure to the sun can lead to premature skin ageing, so you should use suitable sun protection regularly. After a laser treatment, you should avoid direct sunlight for at least 4 weeks. It is important that you wear sun protection with SPF 50+ and reapply several times a day.

How do pigmentation and age spots form?

Even if we know nothing but taut, flawless skin from the media – sooner or later, everyone makes the acquaintance of light to dark brown discolored areas of skin of various shapes and sizes, so-called pigment spots, which are caused by an overproduction of melanin.But moles in exposed areas, pigment spots on the face or décolleté, dark age spots on the hands and shoulders are also an unpleasant cosmetic problem for many of those affected.

Pigment disorders and pigment spots can be found in almost everyone!

The reasons for hyperpigmentation vary: on the one hand there are birthmarks that are already present from birth, on the other hand there are those that only develop over time. These include freckles or pigment spots, which can occur primarily on the upper lip, cheeks and forehead as a result of hormonal factors during pregnancy (chloasma) or are also due to the use of medication. However, pigmentation disorders also often occur with injuries or inflammatory skin diseases. Acne patients are very frequently affected.

Age spots, which usually appear as small dark spots on the back of the hands, arms, shoulders and face, are the result of excessive exposure to ultraviolet radiation from the sun.

What are hyperpigmentations actually?

This refers to an “excess” of pigment accumulation in the skin, especially in areas of the body that are frequently exposed to the sun, such as the forehead, upper lip area, cheeks, décolleté and arms. Depending on the cause of the hyperpigmentation, a distinction is made between age spots, lens spots, melasma and post-inflammatory hyperpigmentation.

Lens spots: These are caused by pigment accumulation due to too much sun or sunburn.

Age spots: From the age of 30, our melanocytes can no longer distribute the pigment evenly in the skin (“ageing phenomenon of melanocytes”), which means that they release melanin more intensively to some skin cells than to others: this is how the unloved first “age spots” appear – typically the size of a lentil and on the face, décolleté and arms (back of the hand).

Melasma: a very special, stubborn form of hyperpigmentation, as the “too much” pigment can accumulate in deeper layers of the skin (dermis). The cause here – in addition to a genetic predisposition – can usually be found in our hormones: hormonal contraception (pill, coil) or a previous pregnancy can be the trigger for melasma.

Post-inflammatory hyperpigmentation: These are small, healed injuries or inflammations in the skin such as pimples, insect bites, etc., which eventually heal into dark pigmentation.

This applies to all forms of pigmentation disorders:

Only clearly benign skin changes are treated with laser, peelings or bleaching cream! (If malignant skin changes are suspected, they are immediately surgically removed and histologically clarified).

What exactly happens during the treatment?

Depending on the diagnosis, peelings and/or bleaching creams are used before the laser treatment, the skin is cleansed and any hair is removed.

When removing individual pigmentation spots, the surrounding skin is covered so that only the pigment is captured by the laser. In the case of extensive pigmentation, e.g. especially in the facial area, a local anesthetic gel is also applied to the skin before the laser treatment.

In the case of superficial pigment – i.e. treatment with ILP lasers – the pigmented area in the skin is broken up into several small pigments so that these can be absorbed by the body’s own scavenger cells and then broken down.

In the case of deeper pigment, fractional CO2 laser technology is used to create tiny wound channels in the skin, which “thin out” the pigment. This results in intensive wound healing and the release of growth factors, which also slightly tightens and firms the skin.

In the case of very dark or deeply embedded pigment, an ablative procedure is used, i.e. pigment is removed step by step in layers using a CO2 laser.

During the laser treatment, the patient only feels the heat of the light. It is not usually necessary to anesthetize the skin. For extremely sensitive skin areas and high sensitivity to pain, an anesthetic substance can of course be applied at the patient’s request to make the treatment as comfortable as possible.

The skin is cooled after the treatment. This soothes it and minimizes any redness that may occur.

Important FAQs on the treatment of pigment and age spots

As the skin is very fresh and sensitive after the treatment, you should avoid extensive sunbathing for a few weeks. In addition, not only should sun creams with a high sun protection factor be used to avoid further UV-induced hyperpigmentation, but also appropriate day creams with a high sun protection factor should be used to keep the treatment results fresh for as long as possible.

To maintain an even complexion after the laser treatment, I recommend an anti-aging cream with natural, brightening ingredients. This is available in our practice!
In my practice I offer a special laser treatment with which freckles can be removed quickly and without scarring.
This always depends on the depth of the pigment. In principle, all forms of hyperpigmentation can be lightened and many can be removed completely.

How do dark circles form?

Treatment with hyaluronic acid injections works reliably here and can significantly reduce dark circles under the eyes.

Dark circles under the eyes can be caused by a lack of fatty tissue (hereditary or due to fat loss as we age) or by hyperpigmentation , which can occur more frequently in darker skin types – a dark shadow appears and is responsible for a tired appearance.

In addition, everyday life also has an impact on the appearance of the skin: stress, lack of sleep and fluids, depression and worry, illness and excessive alcohol consumption can also have a negative effect on the appearance of dark circles.

How does a treatment with hyaluronic gel work?

Using a fine cannula, the hyaluronic acid is injected into the area to be treated. Here, “less” is often “more” to achieve a natural, beautiful result. Depending on the size of the dark circles, further treatment may be necessary.

After the initial treatment, however, it is advisable to wait for the final result before undergoing further treatment after 2-3 weeks at the earliest.

Injections for dark circles under the eyes should be carried out slowly and in several stages, depending on the situation and requirements.
Once the desired treatment result has been achieved, a refresher treatment can be expected after 8 – 12 months, depending on the patient’s tissue. Hyaluronic acid injections also have another advantage: they stimulate the body’s own collagen production in the treated area, so that new and fresh connective tissue can develop in the long term and thus contribute to lasting treatment success. The following applies: the higher the quality of the material used, the better the collagen stimulation!

The distance to further injections of the dark circles will therefore continue to increase.

The injection is carried out in small quantities and in a controlled manner to avoid excessive filling of the tear trough. The treatment is completely safe, as hyaluronic acid is a natural component of our skin.

After the treatment, slight swelling must be expected, particularly in the eye area, and possibly a small hematoma due to the close proximity of the blood vessels. However, both will disappear within a few days and can be easily covered with camouflage. Especially in the area of your dark circles or tear trough, you should only have the treatment performed by specialists in order to achieve an aesthetic and beautiful treatment result.

The result is visible immediately after the injection, as the treatment immediately lifts the dark circles under the eyes. However, the final result can only be admired after a few days.

Due to the presence of a delicate vascular network in the area below our eyes, a hematoma (small bruise) may occur in rare cases, but this can be covered immediately with antibacterial camouflage and usually disappears completely after 2-5 days.

Aspirin or other blood-thinning medication should be avoided one week before the appointment to prevent hematoma formation.

It is important to avoid sports, sauna or steam bath for 2 days after treatment with hyaluronic acid against dark circles and shadows under the eyes and to avoid any external pressure on the treated area – the gel-like consistency of the filler material needs 24 hours to be fixed in the tissue.

Only in rare cases can small but easily concealable hematomas occur in the treated area after treatment of dark circles and under-eye circles with hyaluronic acid. Slight swelling and redness may occur, but this will disappear after 1-2 days.

When treating tear troughs, it is advisable to consult a specialist as it requires a great deal of expertise. In addition to hematomas, which can occur in rare cases, improper treatment can lead to uneven distribution of the hyaluronic acid in the treated area, whereby the excess hyaluronic acid can subsequently be completely dissolved again with a special enzyme.

Here, the relevant expertise of the attending physician is essential. great importance. With Presence of lymphatic drainage disorders (this should be checked by the doctor beforehand – “snap test”), should be a corresponding treatment should be avoided.

After the first session, you can already see a clear improvement in dark circles under the eyes, and a dark shadow can at least be reduced.

A laser lift can be used with the onset of elastosis (initial ageing of the skin due to the formation of tiny wrinkles).

The fractional CO2 laser also leads to a restructuring and firming of the deeper layers of the skin, resulting in overall thicker, youthful and firmer skin.

Immediately after the laser lift there is redness, comparable to sunburn, but this is significantly reduced by applying the post-laser cream. There may also be slight swelling in the eye area, as the skin here is particularly delicate.

Improper treatment can lead to scarring. It is therefore important to have the treatment carried out at a laser-certified practice.

The CO2 laser is a very effective and efficient way to treat scars. A variety of different types of scars can be treated, with the exception of scars caused by self-harm (“scoring”). Dr. Barbara Franz did NOT recommend treatment with the CO2 laser.
